This Expense Voucher is the ONLY authorized form in use. Please do not submit older versions of the form. All vouchers should be emailed to the Finance Chairman at: [email protected]

Detachment Fillable Voucher Form NYS Hotel Tax Exempt Form ST-119.5

Please note: You MUST attach your "NYS Tax Exempt Certificate Form 119" that has the six-digit EX number on it to the ST-119.5 form. The ST-119 form is normally obtained from your Post or Squadron.
